Abstract
Two main criteria are important for the therapeutic management of type I femoral fracture following hip arthroplasty: - Implant stability (prosthetic stability). - Radiographic loosening. Spiral fractures are an indication for surgical treatment (84 %). Healing occured in 80 % of cases within a period of 3 months. Mortality rate is higher in patients who underwent revision arthroplasty (22 %).We recommend the following management protocol:In the case of aseptic loosening : revision arthroplasty with a long-stem femoral prosthesis is indicated.If there is no loosening : internal fixation is preferred. In case of a short oblique fracture, functional treatment can be conducted.
