Abstract
Incorrect Author Affiliation: In the article titled “Effect of Human Papillomavirus on Patterns of Distant Metastatic Failure in Oropharyngeal Squamous Cell Carcinoma Treated With Chemoradiotherapy,”published online March 5, 2015, and also in this issue of JAMA Otolaryngology–Head & Neck Surgery (doi:10.1001/jamaoto.2015 .136), the Author Affiliations were incorrect.
