Abstract
Tibial tubercle osteotomy (TTO) is complex because flat osteotomy is indicated for instability whereas anteromedialization is indicated for osteoarthritis. Instability is associated with patella alta, which sometimes requires distalization, and sometimes pediatric considerations include incompletely ossified tibial tubercle and open growth plates. Complete detachment, as is required for distalization, results in the highest rate of complications after TTO.
