The impact of <i>Pseudomonas aeruginosa</i> airway colonization on lung allografts is not entirely clear. In this issue of <i>Cell Reports Medicine</i>, Dugger et al.<sup>1</sup> identify distinct clinical outcomes and lung allograft biology in recipients with and without cystic fibrosis.
