Abstract
We present the case of primary prostatic synovial sarcoma exhibiting discordant tracer avidity on [<sup>18</sup>F]-FDG PET/MRI and [<sup>18</sup>F]-PSMA PET/CT-a metabolic divergence suggesting clonal evolution and potential eligibility for PSMA-targeted radioligand therapy to combat poor survival in this rare sarcoma subtype.
