Abstract
Vertebrobasilar transient ischemic attacks (VBTIAs) may present with dizziness. Vocal fold paralysis is rarely associated. We describe an 80-year-old woman with recurrent VBTIAs and new-onset left vocal fold paralysis during an episode. The co-occurrence of medullary ischemia and an aortic arch aneurysm illustrates two plausible mechanisms of unilateral vocal fold paralysis.
