Abstract
This article provides background on the impact of Social Determinants of Health (SDOH) and the need to integrate SDOH into the electronic health record for care quality. SDOH screening, documentation, and predictive modeling are all covered. Recent literature is reviewed discussing challenges and opportunities for clinicians and health systems to improve value-based, outcome-driven quality care.
