Abstract
A 53-year-old woman with left lower back pain, anemia, proteinuria, and hematuria was found to have bilateral adrenal masses on ultrasonography. The subsequent contrast-enhanced MRI and 18 F-FDG PET/CT revealed bilateral adrenal masses with locally extensive intravenous tumor thrombi. Postoperative pathology confirmed a malignant perivascular epithelioid cell tumor (PEComa) in the left adrenal region, with tumor thrombi involving the left renal vein, left ovarian vein, inferior vena cava, and right adrenal vein.
