Enhancing Prognostic Communication Skills: Evaluation of the GUIDE E-Module on Illness Trajectories and Prognostic Awareness.

Abstract

Discussing illness trajectories and prognosis is central to advanced care planning (ACP) and a core competency in palliative medicine, yet trainees often report feeling inadequately prepared to lead these challenging conversations. To address this gap, an e-module was developed to teach the GUIDE framework. GUIDE is a structured approach that integrates illness-trajectory graphic aids with communication prompts designed to improve illness understanding and prognostic awareness among patients and families. This feasibility study evaluated engagement with the module, perceived value, and changes in attitudes, knowledge, skills, and confidence among postgraduate palliative medicine trainees. 14 of 20 (70%) eligible trainees completed the e-module and accompanying retrospective pre/post survey. Participants reported improved understanding of illness trajectories, stronger beliefs about the importance of structured prognostic communication, and increased confidence in discussing death and uncertainty. Learners valued the trajectory graphs, demonstration videos, and GUIDE mnemonic and recommended changes for future iterations. Findings support the feasibility and educational utility of teaching about prognostic awareness communication skills using an e-module.
