Arthroscopic Technique for Posterior Labral Repair in the Beach-Chair Position.

Abstract

Posterior labral repair is commonly performed in the lateral decubitus position; however, comparable outcomes have been reported using the beach-chair position. Limited visualization of the posterior-inferior glenoid remains a perceived disadvantage of the beach-chair setup. This Technical Note describes a reproducible arthroscopic technique for posterior labral repair performed in the beach-chair position using strategic portal placement, dynamic portal switching, and controlled anterior translation of the humeral head to optimize visualization. A posterolateral viewing portal enhances exposure of the posterior-inferior labrum, while anterior and posterior portals are alternated as working portals to facilitate suture passage and anchor insertion. Fixation is performed from inferior to superior to restore anatomic labral contour and capsulo-labral tension. This technique preserves the ergonomic and practical advantages of the beach-chair position while maintaining established principles of arthroscopic posterior stabilization.
