Alert and Oriented: Translating Vignette-Based Reasoning Pedagogy into an Open-Access Podcast for Early Learners.

Abstract

Despite the popularity of podcasts as learning tools, publication of student-led, vignette-based podcasts that span the pre-clinical and clinical learning continuum remains underexplored. We describe the development, implementation, and preliminary evaluation of a student-led, vignette-based clinical reasoning podcast, Alert and Oriented, designed for medical learners. Medical students, residents, and attending physicians PROGRAM DESCRIPTION: By translating case-based pedagogical approaches into a podcast format, Alert and Oriented provides medical learners with structured exposure to diagnostic reasoning across three series spanning pre-clinical learners, clerkship-level learners, and physician interviews. Program impact was evaluated through audience reach, episode downloads, and listener engagement metrics. Alert and Oriented has published 59 episodes with 23,655 downloads from 40 countries and 262 cities. Median episode downloads were 244 (IQR, 174-372). Apple Podcast Connect analytics recorded 6042 plays, 453 unique listeners, and 186 (41.1%) listeners meeting Apple's criteria for "engaged listeners," suggesting audience retention. By translating case-based pedagogy into a podcast, Alert and Oriented aims to expand access to diagnostic reasoning resources. Analysis of episode downloads and engagement suggests that this model is sustainable and may be a resource in supporting clinical reasoning development.
