Arthroscopic Free Bone Block Transfer Technique Using Iliac Crest Autograft With Anchor-Button Fixation for Glenohumeral Instability and Glenoid Bone Loss.

Abstract

Bone block transfer for the treatment of glenohumeral instability with severe glenoid bone loss or multiple recurrences has grown in popularity in recent years. In this technical note, we described an arthroscopic-assisted free bone block transfer technique, using a specially designed anchor-button unit to treat glenohumeral instability caused by Bankart lesions accompanying glenoid bone loss. This approach is intended to accomplish mechanical restoration while minimizing subluxation and recurrence, especially in the critical early postoperative phase, through an anchor-button locking mechanism that offers the most significant advantage of this technique: more rigid fixation and improved rotational stability of the autologous iliac crest graft compared with conventional techniques relying on screws and suture anchors.
