Moving toward a Condition-Based Organization - Lessons from the Value Catalyst Hospitals.

Abstract

Many hospitals are working on implementing value-based health care (VBHC), especially through integrated practice units or other condition-based redesign. However, despite the appeal of VBHC, knowledge about the practical processes for its adoption remains limited. The goal of this research is to identify the key dimensions and features of transitioning to a condition-based organization (CBO) and to offer practical guidance for hospitals undertaking this change. A global learning community of 10 hospitals, known as the Value Catalyst, was established, consisting of institutions known and respected for advanced VBHC implementation. During the Value Catalyst learning community meetings, hospitals presented their transition to a CBO; insights from these meetings were used to iteratively refine a framework. Subsequently, hospitals were asked to complete a questionnaire that focused on the implementation of these features within their hospital. The framework includes the following domains: (1) designing the CBO structure, (2) defining roles and processes within condition-based units, and (3) developing a change strategy for the transition toward VBHC. This model is presented together with examples of good practices from the Value Catalyst hospitals.
